You can use the Savings Card at any participating pharmacy
To use your card at a nonparticipating pharmacy: If eligible, we'll reimburse you after you pay for the EFFEXOR XR prescription and mail a copy of the original pharmacy receipt (cash register receipt NOT valid) with product name, date, and amount circled to: EFFEXOR XR Savings Offer, 2250 Perimeter Park Drive, Suite 300, Morrisville, NC 27560. Please be sure to include a copy of the front of your Savings Card, your name, and mailing address
To use your card at a mail-order pharmacy: If eligible, we’ll reimburse you for the cost of your prescription over and above your eligible cost up to the maximum savings amount set forth in the Savings Card Terms and Conditions. Simply pay for your EFFEXOR XR prescription and mail a copy of your original pharmacy receipt (cash register receipt NOT valid) with product name, date, and amount circled to: EFFEXOR XR Savings Offer, 2250 Perimeter Park Drive, Suite 300, Morrisville, NC 27560. Please be sure to include a copy of the front of your EFFEXOR XR Savings Card, your name, and your mailing address. All rebate submissions will be processed within 10 to 14 business days of the date we receive your information
